Paper and online library card applications

Hi, I have a few questions about library card applications. Does your library still use paper applications? For parents with multiple children, do you make them fill out one application per child? Do you offer an online application, and does it have the legal "I'm responsible for my stuff" that they must click to accept? Thanks for the feedback. We still have paper applications, but last winter with our new website we also began offering online library cards. Online apps now account for the majority of our new library cards. We issue them a card number and email that information, along with their 60-day expiration date back to them almost immediately (within about 24-48 hours). We keep the physical card in an envelope behind our circ desk and we double-check all of their info when they come in with their driver's license (or other ID) to get their physical card. Online there is no option for multiple people on one form, so there is no option for multiple children. In person, we do not require them to fill out separate forms for each child, or any form at all actually. We never had any need to refer back to the paper forms and were just recycling them, so we only use them to hand out to a child who is at the library without a parent or someone who just wants to fill it out at home. For children, we use the parent's ID and trust them on the name of their child and the child's birthdate. As for the legality of what we do, I dunno. I should maybe look into that, lol.
