Unfortunately this was not caught before the CD/DVD's were distributed to libraries. Upstart has corrected the problem and the corrections have been uploaded for all of the PDF Forms & Handouts for all 4 programs to the CSLP website. Also several other programming corrections and additions have been loaded to the CSLP website. To find these corrections and additions, please go to the CSLP website at: http://cslpreads.org. Once you log into the site on the toolbar, go to: Membership & Sponsorship --> Member Libraries --> Proprietary Downloads --> 2015 Graphics and Reproducible Files. Each program link directs you to an alphabetical list of all PDF forms from that manual.
