Homebound Delivery/Outreach policy

Hello all! I am looking for insight into library policies from those who do homebound delivery and outreach services. Who delivers the books--does a staff member drive a car or do you have a transport service through your library system or cooperative? How do you handle due dates? Overdues? Do you cap the number of people or residencies you serve at one time? Would anyone be willing to share written policies regarding homebound service? Answers to these questions and any other relevant information would be appreciated! I can also compile answers if other parties are interested.
