Thank you for your responses. I am not the only one that has run into this. Several people have indicated that Baker & Taylor and Ingram have replaced the defective copies upon request. We received an email back from Baker and Taylor yesterday that anyone who bought this can get new copies. They are asking you contact them and then destroy the bad copies. Jenn Donner, MLIS, M.Ed.
