It’s that time of year again! When your summer reading program wraps up, please take a few minutes to complete the annual survey; it provides us with useful feedback: https://www.surveymonkey.com/r/MiSRP17

Adult program, Teens & Children’s stats are all covered in this 1 survey, but you do not need to complete all 3 age groups, just the one(s) you coordinate. Feel free to forward to other departments (childrens/teens/adults) for them to complete their portion. If you’re submitting a district-wide report (in lieu of branch-by-branch), please note that in the “Name of branch” box.

Please submit your responses by Friday, September 8, 2017, to be eligible for a prize drawing.

Questions include:

Total number in age group registered
Total number in age group completed minimum goals
Total number of events specific to age group in summer reading program
Total attendance at said events
Suggestions for 2019/2020 summer reading!
