We charge $0.50/first page, $0.25/page after that, as long as the number is toll-free or within the US.  We provide a free cover sheet that they do not have to pay to fax, unless it's the only thing they are sending.  Our long-distance expenses are pretty low, so it's primarily to cover the cost of the staff time.  It's heavily utilized and greatly appreciated in our community.
