Hello all!
The board would like to evaluate our benefits and see what everyone else is doing, especially in regards to the following questions:

For those offering medical insurance - do you usually go by the cap or 80/20 when it comes to the cost for employees?
We use the State cap and provide three choices. Whatever amount below the state cap that is not used for their selected plan goes into an HSA.

For those offering dental and/or vision - How are you handling this cost? 80/20? Paying in full? etc?
We have two choices, a less expensive that is paid in full or one with more benefit that staff can pay an added amount.

Short Term/Long Term Disability - How are you handling this cost? 80/20? Paying in full? etc?
We pay fully for STD and do not offer LTD.
