I mean, if you want to include gender. I believe the appropriate way to do it is to list:
He/Him/His
She/Her/Hers
They/Them/Theirs

And you should probably consider making it so they can choose more than one.  Some non-binary individuals are comfortable with using more than one set of pronouns.

We don't ask gender on any of our patron forms. It was very easy to remove that question.
