**Please Note** Libraries are listed by districts so it is ok if you are a branch and we receive multiple entries per system. Just please be sure your district is not already submitting on your behalf – we did not have room on the form to list all 645 locations. Thanks for taking the time to respond! The annual end-of-summer survey is now open. Please complete the survey between Monday, July 29, 2019 and Friday, September 13, 2019.
