I’ve had a few requests to see the questions (follow the link or see attached pdf) before completing the Summer Reading survey.

**Please Note** Libraries are listed by districts so it is ok if you are a branch and we receive multiple entries per system. Just please be sure your district is not already submitting on your behalf – we did not have room on the form to list all 645 locations.  Thanks for taking the time to respond! The data and stories are very helpful to CSLP and the Library of Michigan.

Attention Michigan public libraries: please have your summer reading team leaders complete the survey from CSLP (below) even if you did not use the “A Universe of Stories” theme. The survey linked below replaces the individual one Library of Michigan sent out in past years.  It is open through September 13th – I will do a survey prize drawing for some free books relating to summer 2020’s “Imagine Your Story” for Michigan libraries that respond.

Dear Member of the Collaborative Summer Library Program (CSLP),

The annual end-of-summer survey is now open. Please complete the survey between Monday, July 29, 2019 and Friday, September 13, 2019.

The audience for the survey is public library staff who played an active role in administering or running 2019 summer library programming. Multiple library staff from the same library should complete the sections of the survey most relevant to their work; e.g. children, teen, or adult programs.
