Hi Karren,
At TADL we decided to add it to our policy instead of worrying about keeping a zillion forms and worrying about minors checking out items since they legally can not sign a document. When you get a card you agree to all the policies including the liability. Here's the link to the general Circ Policies and  Library of Things Policy.  All are vetted by our lawyer. 

Additionally, we removed any age requirement for any Thing (previously laptops and hotspots only circulated to 18+ years old ) because I thought it could possibly be used against us with patrons asking for certain books/movies/music being circulated by age since we did it for Things. 

Not sure this will work for everyone but it's what we do.
