Hi everyone, I often get asked for our Request for Reconsideration form so I'm sharing it here in case anyone wants to see it. Feel free to use all or part of it. We purposely don't post it on our website (just our policy is there) so that challengers have to actually contact us and ask for it. Our Library Board approved this last summer. I borrowed a little from the forms used by East Lansing PL and Baldwin PL when I worked on it so I just wanted to give them some credit too! Of course, as Clare Membiela always advises, we run everything through our attorney Anne Seurynk.
