Hi Cindy and All:
The Library of Michigan does not have a template, but we do have a couple of info sheets that may be of interest:
Handling
Materials Reconsideration, Challenges, and Censorship, a Checklist
Book
Challenges, Censorship, & Michigan Public Libraries
Both of these have links to resources at ALA and other organizations. I know that the ALA has a materials challenge form template as part of their Selection and Reconsideration Policy Toolkit:
https://www.ala.org/tools/challengesupport/selectionpolicytoolkit
I hope these are helpful!
Sincerely
Clare D. Membiela, MLS, J.D.

I seem to recall during a Library of Michigan's Director's meeting online that there was presented (or suggested) a template of a policy that would cover all of the above. The discussion was
if more libraries had a similar policy it would be easier for interpretation by others. I have been through all of the LM Directors' meeting notes and cannot find anything. Does anyone recall what I am talking about - and/or can you point me in the direction
of obtaining this template? Thanks!
