[Michlib-l] Forms for OCLC in an Academic Library

Hello there, My library is currently updating our procedures and forms for OCLC Worldcat. If any libraries, academic specifically, would be able to share their process and any forms you use, it would be deeply appreciated. The main thing that we're looking for is a good model of a request form that we can keep on hand at the reference desk.
