Hello everyone! We are considering updating our library card application forms, and I was hoping other libraries would be so kind as to share their forms with me, if possible. This would be helpful to determine various ways we might be able to format our applications, while still including all of the critical parts. We still use a physical paper copy for all our library card applications that is kept on record and entered into our circulation program. I am specifically interested in how other libraries handle records releases for minors, or card applications for minors in general, as well as Internet privileges on accounts (for example, we have separate codes in our ILS software based on whether a patron chooses Internet access or not on their form, in order to interact with Envisionware). I know many libraries approach these areas differently, and I am interested in how other institutions handle these aspects. Thank you in advance for sharing this information!
