[Michlib-l] Program recording policy/statement?

Greetings! We are looking for programming policies, statements, permission forms, contract language etc regarding the library's recording of online programs. Has anyone developed a policy to this end? If not, how are you handling performer permission for recordings? Specifically looking for details about which programs are/are not recorded. Where they are made available. How long the recordings are available online. I'm happy to repost information that is sent my way ;) Thanks all!
