[Michlib-l] Paper and online library card applications

Hi, I have a few questions about library card applications. Does your library still use paper applications? For parents with multiple children, do you make them fill out one application per child? Do you offer an online application, and does it have the legal "I'm responsible for my stuff" that they must click to accept? Thanks for the feedback.
