[Michlib-l] Application Form For Library Assistant

Hello Librarians, Does anyone have a template for Application Forms for their entry level Library Assistant sort of positions? Historically, we have only required a resume and letter of interest, but it would be nice to observe what others are using to see whether we may want to modify our procedure in the future.
