
We have a label and a stamp for damaged material that we circ. They are Avery address labels and just say "Damage Noted" with a line to write on at the bottom. (See picture below.) The staff member writes a short description of the damage (coffee spill, crayon, page numbers, etc) and puts their initials and the date.
