We need the help of all Community Anchor Institutions (CAI) in Michigan to answer a short questionnaire regarding broadband adoption. Institutions include K-12 schools, universities, libraries, hospitals/emergency medical facilities, and public safety facilities, and the information you provide data to the Connect Michigan map.

The survey will ask for:

· Name and contact info

· Institution

· Address

· Internet access (Yes/No)

· Internet speed (You can get your speed by linking to the speed test if you don’t know it. This link is found in the survey.) 

It should only take about 3 minutes to complete the survey.
