[Michlib-l] QCD letters to share?

We are a new 501(c)(3) library. I'm learning from a patron that when he does a Qualified Charitable Distribution (donation as an IRA distribution), we are to issue him a QCD letter for his taxes. I have found a couple of samples online, but if you could share your library-specific letter, I would be forever grateful. Have a great day!
