Survey Audience:
The survey is intended for anyone who currently works or recently worked (within the past 12 months) at a public library in the United States and who
is familiar with their library’s cardholder signup process. Your personal information will be kept confidential.
Multiple organizations are distributing this survey; therefore, you may also receive an invitation to participate elsewhere, or you may see
a link for this survey in a library publication or forum. We ask that individuals take this Library Card Access Survey only
once (however we can accept responses from multiple individuals on behalf of the same library).
Participants may provide their name and email address to be entered into a drawing for a chance to win
a $25 (USD) gift card from Giftogram, which allows recipients to select from a list of retailers. One hundred (100) gift cards will be issued.
