[Michlib-l] Library board appointments

Hi!Is there a form of some sort that you use, for a patron to fill out declaring their interest in serving on the library board, that they can give to the township to be appointed to the library board?  Would you please email a copy of it to me? Thank you so very much!!Joann @ Hudson Library
